The Town of Acton is seeking a compassionate and community-focused Clinical Social Worker to join our Health and Human Services team! This role provides assessment, case management, crisis intervention, and service coordination for residents with a primary focus on housing and older adult services. The ideal candidate has excellent communication skills, strong knowledge of clinical social work principles, and proven organizational and documentation skills in a case management setting. This position partners with Public Health, Nursing, Council on Aging, Veterans Services, Building and Fire Departments, healthcare providers, and community organizations to address the social, behavioral health, and environmental factors affecting residents’ health and well-being.
Minimum Requirements: Master’s degree in social work from an accredited college or university. Bachelor’s Degree in Social Work with progressive experience may be considered. Current Massachusetts Licensed Independent Clinical Social Worker preferred. Licensed Certified Social Worker with significant case management experience may be considered. Valid driver’s license required. Three years of progressively responsible experience in clinical social work, elder services, public health, medical social work, home care, behavioral health, or municipal human services is preferred. Experience working with vulnerable adults, interdisciplinary care teams, and community resource coordination is highly desirable.
Pay: $74k-$78k annually.
