Outreach and Transportation Programming Coordinator

Job Description

This position is an outreach and social services role, responsible for all facets of senior services programming including transportation operation of the Swansea Council on Aging and is expected to actively engage with community members. Some responsibilities include application assistance for social services programs, scheduling and reporting for various programs, dispatching, and serving as a back-up van driver when required. Employee provides direction, coordinating with other staff members and supervisor to ensure client needs are met, cost is controlled, equipment is well maintained and the appearance of our drivers and vehicles are up to standards.

The person in this position must also assist with marketing materials such as monthly newsletters and program flyers, and must also complete Medicare Health Insurance Counseling training and counsel Medicare beneficiaries throughout the year as necessary. Position serves as primary outreach contact for the Council on Aging.

• Develop and monitor outreach services. Serve as a resource for seniors seeking health and social services information, providing referrals for resources, navigating service systems, and offering support to ensure access to necessary programs and resources.
• Coordinates and reviews drivers’ tasks and responsibilities, reporting accidents and maintenance.
• Prepare monthly -year end reports required by MArtap, MassDOT, SRTA as required.
